BACKFLOW PREVENTION DEVICE INSTALLATION REQUIRED

A premise isolation prevention device was required to be installed at every water service connection on your property by December 31, 2008. A letter stating this requirement was sent to you in early 2008. Our records indicate that a premise isolation backflow prevention device has not been installed at 123 ANY STREET. This is in contravention of the requirements of Toronto Municipal Code Chapter 851-8 D. Cross connection and prevention. The section states that:

"(4) Unless otherwise required by Section 851-8C or Section 851-8D, every owner of new or existing industrial, commercial, institutional properties and any other buildings, structures and properties that contain a hazard resulting in a risk of contamination of the waterworks as identified in Schedule 5 of this chapter shall install premise isolation desiees in accordance with the standards and specifications. and by the installation date identified in Schedule 5 or upon an order under Section 851-8 D."

You are now required to have an authorized person (as per Schedule 6, of Municipal Code Chapter 851) install and test the device immediately, and submit an owner-signed City of Toronto designated Test Report Form, available on our website www.toronto.ca/water/backflow. A building permit must be obtained before any work under section 8 of the Municipal Code can commence.

Any installed premise isolation devices must be tested and the owner-signed test report submitted to Toronto Water within 7 days of the test date either by email to backflow@toronto.ca, fax at 416-394-5716, or by mail to Backflow Prevention Program, 30 Dee Avenue, Toronto, ON, M9N 1S9.

Please be advised that failure to comply may result in the City of Toronto exercising its enforcement powers under Toronto Municipal Code Chapter 851, as applicable. If you have any questions, please email us at backflow@toronto.ca or leave a message at our Backflow Prevention Line at 416-394-8888. Thank you for your attention in this matter.

Please inform this in writing how and when you plan to address this contravention.
